- NHS Intake, completed, signed and dated
- Disclosure of Programs and Services, signed and dated
- Privacy Policy, signed and dated
- Proof of Income – (1) most recent and consecutive pay-stubs; Social Security/Pension/ Retirement income, provide Award Letter. If self-employed, provide a year-to-date Profit and Loss statement that is no more than 90 days old.
- Bank Statements –Most recent bank statement (should be no more than 30 days old: no missing pages)
- Mortgage Billing Statement- most recent for all loans
- Property Taxes- most recent payment
- Homeowner Property Insurance- most recent statement
